Is this lesson basically telling us to memorize these select phrases? Because it doesn't tell us when else to use imperative subjunctive. How can we know?

The attached covers the imperative conjugations - the only other verb that has “irregular” imperative forms (that is, not directly deducible from the present tense form) is vouloir.

It wouldn't be a bad idea to include that in the lesson as well and have a complete list. Vouloir is used frequently in that form: veuillez patienter (and derivatives).
