I am not sure there should be an extra e at the end of je l'ai regardé as shown in the four options as offered.
Without the link to the lesson this question relates to, it is quite difficult to answer (although Jim's guess is very likely to be the correct answer). Could you add it please?

I assume that the direct object pronoun was grammatically feminine, appearing before the verb regarder and therefore requiring concordance with the past participle.
