Bonjour Sydney,
Que dessinent les enfants ? = What are the children drawing?
Que dessinent-ils, les enfants ? = What are they drawing, the children?
As you can see the second example is slightly different from the first.

Yes, you could either say:
Les enfants, que dessinent-ils? or Que dessinent-ils, les enfants?. Note, however, the comma separating the pronoun from the explicit subject.
