How would you say "You went out even though I wasn't OK with it." ?

This correction confuses me, because in the English phrase the past tense is used (I wasn't OK with it). Always remember that after the expression "bien que", the verb will be conjugated in Le Mode Subjonctif.
The first 3 options you are mentioning are all conjugated in Le Mode Indicatif or Le Mode Conditionnel. Therefore, they cannot be correct. Only the option with Le Subjonctif is correct here.
Therefore, after "bien que", the French translation will not always match with the tense in English, but rather remember to use Le Mode Subjonctif:
1. Le Subjonctif Présent if you are/the person talking is considering the action in the second clause (i.e. the "bien que ..." clause) as happening simultaneously with the main action
or
2. Le Subjonctif Passé if you are/the person talking is considering the action in the second clause as having been completed beforehand.
