Vous and plural form of verb

In two examples (one below), it appears as if the "vous" is plural and not singular (formal). The other instance appears in the examples. Should the use of "vous" be assumed to be plural without any clarifying context? And, if "vous" were singular (formal "you") in this sentence, would the verb form be "étiez couché"?

Vous ________ tôt ce soir-là.

You had gone to bed early that night.

Answer: vous étiez couchés

Vous vous étiez couchés tôt ce soir. (addressing a group)
Vous vous étiez couché tôt ce soir. (single person, formal)
