Ça vs il

What's the rule here, i.e. how can we now it is not "il me dérange quand on fume..."

This question relates to:
French lesson "Ça = that, this, it (pronoun)"

Thanks for the lesson link, Aurélie, but that lesson seems to be about "c'est" vs "il est". "ça" is a third alternative, right?
