Why is the plural "vos" used instead of votre? What part of this sentence is plural? I thought this was the formal "you" not plural

It actually stops you repeating votre twice so -
nom et pronom.
The speaker viewed nom et prénom as two "names". Hence, probably, the plural. I don't think it would be wrong to say ...votre nom et prènom.
