Not at all : "Où vas-tu ?" is the inverted question form, which is actually considered more elegant and formal than its equivalents:
"Où est-ce que tu vas ?"
"Où tu vas ?"
"Tu vas où ?"

That is not considered wrong, it is just another way of saying "where are you going"
Comment tu t'appelles means what is your name and Comment t'appelles tu also means what is your name,the only difference is tu comes before t'appelles in one of the statement. So vas coming before tu in one of the sentence does not make it incorrect ,it is just another way of saying it.
