Taken from a recent exercise -
Il s'y connait en mécanique = He is very good in mechanics
I can't think of an example with 3 pronouns (but doesn't mean it is impossible) although the meaning might be lost so unlikely to happen.

For an example that uses y as well as en: Il y en a deux. -- There are two of them.
J'y en ai posé deux. -- I put two of them.
But I don't think that's exactly what you were looking for.
I think Diego meant y-en-verb . . . and I think he meant as double pronouns that precede the verb. I don't think that's possible, but there are still a lot of things I don't know about french. The question seems to be one more of curiosity than usability.
