This is the first lesson I can't work out the difference between the two structures something + "plaît à..." versus the reflexive something + "me plaisent."
Both descriptions say they are to like something, I can't work out when to use which structure. I've re-read the lesson about 3 times, so I'm looking for additional clarification..

…is cited as an example of when it’s not appropriate to use the subjunctive.. Fair enough, but why is it also included amongst those examples that do use the subjunctive? Is that an error or am I missing something?
I agree with the last two questions. 'Elle est' seems correct, as it is a description of a specific person.
The narrator is recalling events that happened on only one occasion and instantaneously. Why does she use the imperfect?
I think it would be helpful to see more test questions using lorsque, après que and une fois que rather than just 'quand' in the Futur Antérieur.

je ne veux pas du tout lait ?
does the 'du tout' cancel out the de ?? (normal sentence: je ne veux pas de lait)
also regarding verbs that need prepositions before the infinitive...
je ne te permets pas du tout de venir ici
or
je ne te demande pas du tout de me voir
